Note on front:  This is the Drs Mayos Hospital (am assuming that it is referring to the Mayo Clinic).

Note on back:  Why did you not come up here ___ me to have your operation.  They do about 25 a day here.  I go to the clinic tomorrow morning 8am.  They operate from 8 until 10.  This is a very pretty little town about 8000 population
